Important information for Healthy Options Enrollees

Washington State has chosen new insurance carriers for Medicaid and Healthy Options patients.

CHCW has signed contracts with Molina and Coordinated Care. If you have been incorrectly assigned to another plan or provider other than one at Community Health of Central Washington (YPA, NMC, CWFM or CHCW-E), you can ask to be reassigned.

We are happy to help you make this transition so you remain with your current clinic and provider. You can also download the forms below to make the change yourself. Changes become active the month following your request, but we will continue to see you prior to that if a need arises.
