firststepFirst Steps Program

Pregnant women, new mothers, and babies are eligible for the First Steps Program when Medicaid provides their health insurance. We provide home or office nursing visits for assessment, referral and health education through the second month after the baby is born.

Infant Case Management allows us to follow infants who qualify up to one year of age. We provide referrals to resources for infants who meet certain criteria.

Children and Youth with Special Health Care Needs

The program promotes an integrated system of services for infants, children and youth up to age 18 years who have or are at risk for chronic physical, developmental, behavioral, or emotional conditions and require health and related services of a type or amount beyond what is generally needed.

The nurse is able to help families navigate through complex medical systems and connect with resources.
