Our Services
Community Health of Central Washington pharmacies are open to all CHCW patients! A pharmacist is available for consultations on diabetes, Warfarin, and other chronic conditions.
Our pharmacists work with providers at the Yakima and Ellensburg locations (including pediatrics). Clinical pharmacists are a part of your care team, and are available to meet with you and/or your provider. They specialize in managing medications to make sure they are helping you stay well.
Comprehensive Medication Review
Pharmacists meet face-to-face or via telephone with patients to perform comprehensive medication reviews of all medications and disease states, identifying medication-related issues and creating action plans and recommendations to resolve these issues.
Warfarin Management
Often patients on the path to recovery from strokes or heart problems are prescribed anticoagulation treatments, like Warfarin. Anticoagulants are medicines prescribed to slow down the body’s own clotting process and to keep the blood thin.
At Community Health of Central Washington, we’ve established our Warfarin Management Program to serve the specialized testing, monitoring and prescription needs of patients undergoing treatment with Warfarin (Coumadin). This program has greatly enhanced the safety and proper dosing of medication for all patients enrolled. A simple “finger-stick” is all that’s needed to obtain the small amount of blood needed for analysis during each visit to the clinic.

Sign Up For Our FREE Prescription Alerts! Our FREE Prescription Alerts give you the confidence that we’re taking care of your prescriptions to prevent wasted trips and surprises. When your prescriptions are ready, you’ll get a text or phone call letting you know. And, if there is a delay, we’ll keep you informed of that as well.
Prescription Financial Assistance
As a patient of CHCW, you may qualify for financial assistance with your prescriptions.
Prescription Access Programs:
To ensure that our patients can afford their medications prescribed, CHCW may offer an eligible patient discount to those who qualify and are 200% below the Federal Poverty Guidelines. You can access this discount fee application below to apply:
