Our Primary Care Behavioral Health (PCBH) is a regular part of your primary care team and communicate regularly with your primary care provider (PCP).
Your PCP and PCBH work side by side to deliver cutting edge healthcare services. Our PCBH team use behavioral science to help our patients develop a plan to improve their overall health as well as to support them in living a meaningful life!
How To Get Started
We work directly with your Primary Care Physician (PCP) at CHCW. To see a PCBH, you can simply ask your PCP and a PCBH will see you that same day or you can call to schedule an appointment! If you do not have a PCP yet, or you want to transfer to CHCW, we have a lot of locations to serve you.

In addition to having some PCBH who are Spanish and English bilingual, we also provide virtual interpreter services, including Sign Language. We are here to make you comfortable, in any language you prefer, just let us know at your next appointment!
